MPH in Public Health

If your academic records are not in English, please also submit an ATA certified English word-for-word translation of your documents to the credential evaluation agency.

International applicants (those requiring a F/J visa or I-20 to study in the US) applying to a graduate/postbaccalaureate program with non-US academic records/transcripts are required to submit their official foreign academic records (with the institution's grading scale) to International Education Research Foundation (IERF) or World Education Services (WES) for a Course-by-Course, Detailed Transcript Evaluation Report.

A bachelor's degree from a regionally accredited institution.

Unofficial transcripts from all colleges or universities attended, including Cal State LA. Official transcripts for bachelor's or professional degrees obtained, as well as certificate programs in which letter grades were awarded.

Financial Sponsor information and a copy of their recent bank statement (or other official letters of financial support/guarantee/scholarship).

Two reference forms: One reference must be academic; the second may be academic or professional. If the last degree was earned over five (5) years ago, two professional references are allowed.
